Sports physical exams to be offered Thursday, July 18

Promotional image for fall sports physical examsStudents interested in participating in fall sports can fulfill their physical exam requirement this Thursday, July 18, starting at 9 a.m. Physical exams will be conducted on a first-come, first-serve basis by Dr. Jeff Horowitz at the Warwick Valley High School, in the nurse’s office. 
Parents, please complete and sign a medical history questionnaire (English / Spanish) and have your student bring it to the physical examination. This form is part of the physical and serves as a permission for participation in sports. 
To help you determine if your students needs a physical and to schedule one on Thursday, July 18, please contact Maggie Adams RN, MSN, at 845-987-3050, ext. 12710 beforehand. 

About physical exam requirements

Athletes must have a current physical exam on file in the nurse’s office prior to the start of the sports season. Physicals are only valid for one year. For varsity/JV sports, physical exams must have been completed no earlier than Aug. 1 of the current year to be considered current. For modified sports, physical exams must have been completed no earlier than by Sep. 1 to be considered current.
