The procedures for obtaining evaluation information include:
- contact the building principal regarding your interest in receiving principal or teacher evaluation scores;
- request the information in writing or schedule a meeting to receive information in person; and indicate your child’s name and grade.
Suggested language for requesting evaluation scores in writing:
Dear Principal,
As stipulated in the New York State Education Department regulations, I request to receive my child’s current teacher and/or principal evaluation scores.
I understand that I may receive the scores of current teachers and principals of my child only.

I request the following information:
____ Teacher evaluation score and rating
____ Principal evaluation score and rating
____ Explanation of the scores and rating
