Athletics Preliminaries & Physicals

In the coming weeks, the school nurses will begin conducting “preliminaries” for Sports Physicals for the 2023-2024 school year for  students in grades 6 through 11 who wish to receive a sports physical conducted by the school physician.

If your child will be seeing their private physician to obtain a physical for the upcoming sports seasons and you do not wish to have one conducted by the school physician, no further action is required.

Preliminaries are performed by the school nurses and are completed before the formal physical examination. 

Preliminaries include the following:

1. Completed Spring 2023 Medical History Questionnaire (MHQ) (completed by the parent/guardian)
2. Blood pressure
3. Pulse
4. Height/Weight

The Spring 2023 Medical History Questionnaire can be found on Operoo:

1. Log into Operoo by clicking the link here.
2. Select “Form Library”
3. Select “Spring 2023 Sports Physical Medical History Questionnaire”
4. Complete the form by 3/1/2023 for your child(ren).

Please note that your child(ren) will ONLY be scheduled for a physical if the Medical History Questionnaire is completed by 3/1/2023.

The next step in the process is the Sports Physical. The exams will be completed in March, April, May, and June. Physicals are conducted in the nurse’s office by the school physician during the school day.

Health Requirements for Participation in Interscholastic Sports 

For your child to be eligible to participate in interscholastic sports, the WVCSD Health Department requires the following documentation to be submitted BEFORE the start of tryouts.

• A physical completed within one year of the start of the sports season in which your child will be participating. 
  • Physicals must be completed in their entirety on the official NYS form and signed by the provider that completed the exam.

• NYS Interval Health History must be completed before each athletic season

• 2022-2023 Medical History Questionnaire. 

• If your child has had COVID, COVID Cardiac Clearance is required to participate in sports.  
  • Please note that your child will need to be cleared each time they have COVID.

• Ex. If your child has had COVID, recovered, obtained cardiac clearance, and has not had COVID since that clearance, they do not need to be cleared again to participate in sports.

• Ex. If your child has had COVID, recovered, obtained cardiac clearance, and tests positive for COVID again after that clearance, they are required to obtain a new Cardiac clearance to be cleared to participate in sports. 

Required Documentation:

• The 2022 Medical History Questionnaire will be available for completion in Operoo beginning August 1, for current students. 
• All other sports forms will be available for completion in Operoo beginning mid-August.
