2023-24 Wildcats Athletics Physicals
WVCSD Medical Director — Dr. Jeff Horowitz — will be at Warwick Valley Middle School on Thursday, August 3, 2023, to conduct required sports physicals for students who plan to compete in the fall Wildcats athletics season.
Step 1: Schedule your appointment.

If your student needs a physical to participate in the 2023 fall season, please use the link below to schedule their appointment. Spaces are limited, and walk-ins will not be permitted.
Step 2: Complete your medical history questionnaire.
Log in to Operoo and complete the 2023-2024 Medical History Questionnaire by August 1, 2023. You must complete this form in order to keep your scheduled appointment time.
If you need assistance with your Operoo account, email helpdesk@wvcsd.org, and write “Operoo” in the subject line. A member of the tech team will contact you.
Step 3: The day of your physical.
- Students should be dropped off/picked up at the WVMS entrance on the day of their physical. WVHS is currently closed for ongoing renovations.
- Proceed to the WVMS Nurse’s Office to check in for your scheduled appointment.
