sports physical information
In addition to a
yearly sports physical, a Sports Physical Update Form is due
from each prospective athlete PRIOR to participation in each
sport season. The Sports Physical Update Form (or Interval
Health History Form) is available from your coach or in the
nurse's office. It must be returned to the school nurse prior
to the start of try outs. If the update form is not turned
in and the physical is not up to date, the student will not
be on the eligibility list for sports and will not be
allowed to participate.
