Immunization
schedule for entering students
NYS law requires children entering school to have the following immunizations:
| Oral Polio | 3 or more doses |
| Measles vaccine | 2 doses |
| Mumps vaccine | 1 dose |
| Rubella | 1 dose |
| Diphtheria toxoid | 3 or more doses |
| Hepatitis B | 3 doses |
| Varicella | 1 dose for
students born on or after 1/1/98, or born on or after 1/1/94 and entering grades 6, 7, 8, 9 or 10. |
| Tdap vaccine | 1 dose for students born on or
after 1/1/94, entering, repeating or transferring into 6th gr., and students in grade-less classes born on or after 1/1/94 and age-equivalent of 6th gr. |
